After An Evaluation, How Does A Patient get Their Cannabis Card?

After getting an evaluation and receiving a certification form from the provider, the patient will have to apply to the Iowa Department of Public Health either online or by mail.

How Do I Renew My Certification?

By Iowa law, a patient certified by their provider for the Iowa cannabis program must be reevaluated and recertified yearly.

Currently, I'm providing Telehealth evaluations for patients across the state of Iowa.

Refund Policy

A full refund will be made if the patient cancels their appointment 24 hours ahead of the scheduled time or if the provider must cancel the appointment. A refund cannot be given if the patient cancels the appointment without a full 24-hour notice.  

In the event of unforeseeable technical difficulties or disruption of evaluation on behalf of the clinic, the patient will be allowed to reschedule the appointment. No refund will be given. If there is technical difficulty or disruption on behalf of the patient, a refund will not be given.    

A refund will not be given in the case that the patient makes an appointment but does not meet the qualifying conditions for the Iowa medical cannabidiol program and/or cannot be recommended for medical cannabidiol registration.
